What Is an EHR? History, Laws & Certification

EMR vs EHR vs PHR, how EHRs became universal (HITECH, Meaningful Use, Promoting Interoperability), ONC certification, the Cures Act and information blocking.

EMR, EHR and PHR

Slide 1 / 5

• EMR (electronic medical record) — a digital version of one practice's paper chart; mostly used within that organization. • EHR (electronic health record) — a broader, interoperable record that shares information across organizations and settings, with decision support and patient access. • PHR (personal health record) — maintained by the patient (e.g. a phone health app pulling data from providers).

In everyday use, people say 'EHR' for both EMR and EHR.
