Guided Notes

Health Information Exchange

How records move between organizations — C-CDA documents, Direct messaging, query-based networks (Carequality, CommonWell, eHealth Exchange), TEFCA, HIEs, and patient matching across systems.

Three ways to exchange

Slide 1 / 5

• Directed exchange ('push'): send information to a known recipient — e.g. a referral or discharge summary via Direct secure messaging. • Query-based exchange ('pull'): search for a patient's records across organizations when they show up for care. • Consumer-mediated exchange: the patient retrieves and shares their own data (portals, apps).

Most organizations use all three.
