Injury coding rules
Slide 1 / 6
• Code each injury separately unless a combination code exists.
• Sequence the most serious injury first, as determined by the provider and the focus of treatment.
• Superficial injuries (abrasions, contusions) aren't coded when they're at the same site as a more severe injury.
• A primary injury with damage to nerves or blood vessels → code the primary injury first, then the nerve/vessel injury (or vice versa if that's the focus).
• 7th characters: A = active treatment (including surgery, ER, evaluation by a new physician); D = routine healing/follow-up; S = sequela.
